Report a Claim
The information below is intended to assist member agencies with reporting claims under CIRA’s coverage programs. Prompt reporting helps facilitate timely investigation, efficient claim handling, and compliance with applicable reporting requirements.
Please email claims@cira-jpa.org or reach out to a member of CIRA staff with questions or if you need any assistance.
To report a General/Auto Liability claim, email all forms and documentation received from the claimant immediately upon receipt to the claims administrator:
To: MyGHCNewClaims@georgehills.com
Cc: claims@cira-jpa.org
Once submitted, you will receive an email confirming receipt. The claim will then be assigned to an adjuster, who will contact you directly for further processing.
To report an Employment Practices claim, IMMEDIATELY provide written notice to ERMA using the ERMA Initial Report Form with the following information:
- Member entity (name of City/Town, member of CIRA)
- Identity of the claimant employee(s) and the identity of any accused employee(s)
- Time, place, and circumstances surrounding the incident
- Names and addresses of witnesses (if any)
- Any other relevant/requested information
Submit this form to Stacey Sullivan, ERMA Litigation Manager:
To: stacey.sullivan@sedgwick.com
Cc: claims@cira-jpa.org
ERMA will respond with additional instructions for claims handling.
*Important Billing Reminder*
Effective July 1, 2025, all invoices and billing from investigators or attorneys related to ERMA claims must be sent directly to ERMA for payment processing at ermainvoices@sedgwick.com. Do NOT pay any invoices directly. You may be copied on correspondence for your records, but this is for your information only – no action or payment is required on your part.
To report a Workers’ Compensation claim, submit an Employer’s First Report of Injury (Form 5020) using LWP’s secure loss reporting portal. Review the LWP Instructions for Online 5020 for step-by-step instructions. Upon receipt, claims will be assigned to an adjuster, who will coordinate with you directly to continue processing. Review the Workers’ Compensation Program page for important reminders and helpful resources. If you have additional questions, contact Monica Verdugo-Thomas, Claims Supervisor, at (916) 609-3617.

2) Property Losses Between $25k – $50k (Excluding Auto)
⚠️ Important Reminders Before Submitting a Claim:
- These procedures do not apply to auto (vehicle) losses — only property damage between $25k and $50k.
- All claims must be submitted to the Program’s insurer for adjusting as soon as possible, but no later than one (1) calendar year from the date of loss.
✅ Step 1: Complete the PRISM Property Loss Report
Upon receiving notice of a property damage loss valued between $25-$50k, complete the PRISM Property Loss Report and submit it to the designated claims team noted above.
